The Church Hill Medical Mission, a clinic operated by Of One Accord Ministry, is committed to providing high quality care with dignity. CHMM is proud to share that they have earned a 2021 Gold Rating from the National Association of Free and Charitable Clinics (NAFC) Quality Standards Program!
The NAFC ensures the medically underserved have access to affordable health care. To quantify and qualify the care provided at the Free and Charitable Clinic network, the NAFC formalized a set of Quality Standards for member organizations.
NAFC members, such as CHMM, submit information to the NAFC that demonstrates the quality of care provided. NAFC Quality Standards elements include policies and procedures related to the following areas: Administrative, Enhanced Access and Continuity of Care, Identifying and Managing Patient Population information, Planning and Managing Care, Providing Self-Care Support and Community Resources, Tracking and Coordinating Care, and Measuring and Improving Performance.
CHMM is thrilled to have received this top rating which highlights their commitment to providing high quality care for their patients.
This is the second year in a row CHMM has received this award.
Last year, CHMM treated 536 patients and in 2021 they have treated 210 patients so far. The clinic provides general medical care and specialized diabetes care to under and uninsured residents in Hawkins and Hancock County, TN and Scott County, VA.
