Eligibility Guidelines:
| We serve patients who are uninsured or underinsured and whose incomes fall within 185% of the federal poverty level. Please click here to determine if your income is within these guidelines. |
| You do not need to bring any proof of income to your first visit to the clinic. |
| You will be given an eligibility application at your first visit or you can print one by clicking here. |
| Please complete the application and bring it, along with proof of your income to your scheduled eligibility appointment or you can mail it along with proof of income to Church Hill Free Medical Clinic, P.O. Box 166, Church Hill, TN 37642. |
| You application will be processed and a letter of determination will be sent to you before your next visit. |
Diabetes Clinic & Other Services:
Patients must be seen at our evening clinics before being scheduled for follow up at our diabetes clinics or for other services.
Clinic Schedule:
The clinic is open during the second and fourth weeks of each month on Tuesday and Thursday evenings at 5:30 p.m. Please see schedule (English version) (Spanish version) for more details. See provider schedule below.

Patients are seen on a first-come, first-served basis, limited to the first 30 patients. Although the clinic opens at 5:30, we recommend you try to be there before 5:00 in order to be seen.
Cost for Services:
The clinic does not charge for its services, but does accept donations from patients and area churches, businesses and organizations.
