Eligibility Guidelines:

	We serve patients who are uninsured and whose incomes fall within 185% of the federal poverty level. Please click here to determine if your income is within these guidelines.
	You must apply to become a patient at the free clinic. Eligibility applications are processed on the first Thursday of each month from 10 a.m. to 2:00 p.m.
	Applications can be picked up at the clinic on Monday through Thursday from 10 a.m. – 5 p.m.  However, we ask that you call the clinic in advance at 423-256-2408 to make sure someone is in the office.

Diabetes Clinic & Other Services: Patients must be seen at our evening clinics before being scheduled for follow up at our diabetes clinics or for other services.

Clinic Schedule: The clinic is open on the second, third and fourth Tuesday evenings of each month from 5 – 8:30 p.m.  Doctors start seeing patients at 6 p.m. Please see attached schedule (English version) for more details. Patients are seen on a first-come, first-served basis, limited to the first 25 patients.

Cost for Services: A $5 donation is requested, but not required.
